Project Overview
Atrial Fibrillation, Ipswich and East Suffolk Remote digital monitoring project is to test the acceptability and feasibility of identified populations self-monitoring via a FibriCheck application and wearable single lead ECG, Zio XT Patch whilst providing a suitable pathway for treatment, without adding additional pressure on primary care.
This project is led by Suffolk and North East Essex (SNEE) Integrated Care Board (ICB) and will be tested at Ipswich Hospital.
Eligible patients will be identified using the Eclipse system and receive digital engagement via SMS text. This will guide them to a lander page which will explain the programme and guide them to a secure NHSConnect portal where they can complete a questionnaire and consent for participation within the programme.
Patients will then be instructed on how to activate and use the free FibriCheck application. The results from the FibriCheck application (uses photoplethysmography sensors to determine the heart rhythm of a user) will be viewed via the secure Fibricheck portal by trained members of the Ipswich Hospital cardiology team.
The objectives of the programme are:
Improved detection of AF in at risk population and reduced time to access treatment
Reduction in number of strokes in people with known AF
Reduction in demand of ECG undertaken within primary / secondary care
Reduction in face to face attendances, due to at home-monitoring systems
Improvement in patient experience
