Black Pear integration with your clinical system and Eclipse
Pro-Forma
Eclipse is collaborating with Black Pear to enable the transfer of patient data from Eclipse to your clinical system.
Using Black Pear's IM1 Assured Core Service, this enhanced functionality is now available to your practice. Black Pears Core Service uses smart reconciliation technology to make relevant updates directly to patients’ records, enabling clinicians to choose the updates they need. Enabling Eclipse to seamlessly add patient information, reports, and clinical codes directly from a patient's care plan into your clinical system, whether it's EMIS or SystmOne.
Eclipse allows:
Identification of patient cohorts
Risk-stratification of patient cohorts
Engagement with patient cohorts
Data and monitoring optimisation of patient cohorts
Facilitation of clinical review
Identification of target implementations
Personalised care plan creation
Implementation of care plan
Follow-up of patients within a cohort
The simple integration process described below will allow clinical coding, patient engagement and pathway activity, review outcomes and care plans to be inserted into the patient’s medical record on the clinical system.
SystmOne (TTP) Integration:
Ten-minute call with one of your team members (usually a member from your IT team) to discuss practice system permissions as the process will require elevated privileges. Your surgery consent will be gained via completing this form (this ensures that all IG/DPIA is in place to proceed, in line with your existing IG contracts).
Integration must be deployed to a dedicated Microsoft Windows unattended workstation, (Gateway machine). We will be in touch with pre-requisites for the call.
EMIS
To configure the new service, we would require your ODS Code and CDB number (organisation ID). We will then work directly with EMIS to activate the service for you. No further contact with the surgery IT department is needed. This will be requested following receival of this completed form.
