The Community Pharmacy Support Programme pilot is being launched across the South West London and South East London ICBs. This programme offers your practice the opportunity to streamline patient care, enhance patient safety, and reduce workload through automated digital engagement with patients eligible for Luteinising hormone-releasing hormone (LHRH) treatment. By transitioning from LHRH administration to Gonadotropin releasing hormone (GnRH) by a trained pharmacist, the programmed aims to alleviate the burden on nursing staff and GP practices.
Programme Overview
The programme’s objective is to streamline patient care and enhance patient safety for prostate cancer patients receiving LHRH treatment, while reducing the primary care workload across the region.
Key Components of the Programme
Identification of Eligible Patients: Using the NHS England assured Eclipse Clinical Support Platform, eligible patients will be identified and prioritised for LHRH treatment. Patients will be grouped within the Eclipse pathway according to their responses and the next action required, such as scheduling their injection at a community pharmacy.
Digital Patient Engagement: Patients will be engaged through automated digital communication, receiving SMS messages that link to an informative programme page. This page will explain the benefits of receiving their LHRH injections at a participating community pharmacy of their choice in their area and provide detailed information about the process and patient journey.
Secure Questionnaire: Patients will complete a secure questionnaire via the portal to confirm their current symptom control and express their treatment preferences.
Community pharmacy coordination: Eligible and consented patients will be booked in for an appointment at their community pharmacy, where they will be able to receive their treatment from trained community pharmacists for LHRH injections.
Programme surveillance: Patients will receive follow-up communication post-engagement to prompt them to access additional support services, ensuring they are comfortable with the new arrangement and to address any concerns they may have.
