25 million patients that have an active Eclipse portal. Supporting at risk patients through integrated technological enabled care services. The GaitSmart Eclipse Connect service enables:

  1. Central identification of patients at-risk within these pathways, including utilisation of the standardised electronic Frailty Index (eFI) score.

  2. Standardised Communication with all these patients to engage them via an onlinepatient portal, supported by a specialist engagement team for those patients that are digitally deprived,, with questionnaires to gather on-going patient feedback and insight.

  3. Bespoke communication with vulnerable patient groups, ensuring equality of service access.

  4. Evaluation of the feedback to enable enhanced reviews by the service and/or local clinical teams.

  5. Ensure patient appropriately investigated and any background risks addressed.

  6. Provide defined vulnerable / at risk patients with additional support and monitoring

  7. GaitSmart assessment programme and other self monitoring capability to minimise risk and optimise patient wellbeing.

  8. Prioritisation of the Patients into 4 distinct groups.

    1. Confirmation that specialist falls and frailty services are required.

    2. Gait anomaly identified or other clinical risk identified within the service that may respond to social prescribing and/or GaitSmart Standardised Assessment and Individual Exercise plan.

    3. Significant clinical risk prioritising referral to an alternative pathway (locally defined) patient insight and feedback enabling removal from referral pathway waiting list.

    4. Validation of impact including equality of care analysis of the CONNECT programme.

    The system undertakes a Gap Analysis to identify missing information or patients in need of a review. It also enables prioritisation of patients through standardised criteria. Patients from vulnerable cohorts and lower socio-economic locations can be identified to enable appropriate support implementations.
