Population Health Intelligence for the NHS

Transforming NHS Data into Better Patient Outcomes

Eclipse helps NHS systems, neighbourhood teams and GP practices identify priority patients, focus finite clinical capacity, engage people equitably and evidence measurable outcomes.

Eclipse is a clinically led NHS population health intelligence platform supporting proactive care, medicines optimisation, patient engagement and outcomes validation at scale.

From Integrated Care Board oversight to GP practice action, Eclipse helps teams move from broad population insight to targeted support for the patients most likely to benefit.

Why is Eclipse Unique for ICBs?

Empowers NHS organisations with insights from the largest NHS centralised intelligence platform

Provides secure, system-wide access across ICBs, NHS Trusts, PCNs and GP surgeries

Uses billions of weekly data signals for early identification of high-risk patients

First-in-kind NHS AI platform with zero patient data exposure to LLMs, powered by 2,500 risk parameters for personalised risk stratification and reporting

Enables dynamic and scalable patient engagement across entire care systems

Delivers AI-powered personalised patient education and self-management

Identifies over £30 million in annual prescribing savings for each ICB

Proven to reduce population-level emergency admissions by 10% and high-risk patient cohorts by 30%

Live Patient Records

The Advice and Guidance (Eclipse Live) service gained NHS England central assurance in 2017 and is being supported by NHS central funding. The service has been commissioned by more than 3,300 GP practices and 23 ICBs and is currently actively risk stratifying more than 27 million patients and continues to expand across the NHS. Regions utilising the service benefit from the creation of a dynamic centralised patient database containing individual-level clinical data information and proactive risk stratification.

Enabling Proactive, Preventative & Personalised Care at Scale

Eclipse is a clinically led population health intelligence platform, developed within the NHS to help Integrated Care Boards move from reactive care to earlier identification, prioritisation and coordinated intervention.

By securely bringing together data from primary, secondary, community and patient-derived sources, Eclipse helps systems identify risk, reduce unwarranted variation and improve Equality of Care across whole populations.

At its core is Eclipse RADAR, an NHS Digital-assured prescribing and patient safety risk stratification service. Alongside medicines optimisation, patient engagement, pathway oversight and outcomes measurement, Eclipse gives ICBs the insight needed to reduce avoidable deterioration, demonstrate impact and deliver safer, more personalised care at scale.

PROACTIVE CARE

Eclipse is committed to the proactive management of high-risk patients.

SUSTAINABLE HEALTHCARE

Creating sustainable healthcare through workforce optimisation.

EQUALITY OF CARE

Ensuring equality of care is delivered across all populations.

PATIENT EMPOWERMENT

Empowering patients to be more involved in their care decisions.

OBJECTIVE IMPACT

Healthcare systems have fully integrated data parameters to objectively calculate impact.

SECURE DATA

Creating the most secure data environment in healthcare.

How ECLIPSE Works

ECLIPSE is not simply a reporting platform. It provides an end-to-end operating model for identifying opportunity, activating programmes, prioritising patients, engaging populations and validating outcomes.

  • Centralised Population Health Database

A secure, centralised population health database brings together relevant data from across the system, creating a single foundation for insight, prioritisation and coordinated action.

  • Identification of ICB Opportunities & Priority Pathways

Population-level analysis helps identify the greatest opportunities for improvement, highlighting priority pathways, unwarranted variation and areas where targeted intervention can deliver the greatest impact.

  • Programme Activation & GP Proformas

Priority programmes are translated into practical action through structured activation, including GP practice tools, clinical workflows and proformas that support consistent delivery at scale.

  • Patient Identification & Risk Stratification

Patients are identified and prioritised using a combination of clinical, operational and population health intelligence, helping teams focus attention where intervention is most likely to improve outcomes.

  • Dynamic Patient Engagement

Targeted engagement supports patients through personalised communication and outreach, helping improve uptake, participation and the effectiveness of interventions across different population groups.

  • Validation of Outcomes & Continuous Improvement

Outcomes are measured and validated over time, enabling systems to demonstrate impact, refine delivery and support continuous improvement across pathways and populations.

“This was a worthy winner due to the inspirational and person-centred model of development, originally in response to the Covid-19 crisis, which later evolved into meeting the needs of the wider health and care system, the move from standard medical model to all aspects of the care of a persons outstanding. Panels strongly recommend that other care systems study and adapt to this approach in developing their own solutions to meet the needs of the local population. And the winner is Norfolk and Waveney CCG.”

HSJ Expert Panel

“It was a good example of work and health and social care and the borough councils and the prescribing team working together. Primary care appreciate that the patients clearly valued it because there was a lot of social support that this project was able to identify that met the needs of these patients to enable [them] to have better outcomes.“
