Blood Pressure Protect enables your ICB to centrally support practices in identifying and optimising care for individuals with hypertension or raised blood pressure who may be at increased risk of serious complications. By supporting proactive review, targeted intervention and improved management, the programme helps reduce the likelihood of avoidable cardiovascular events, emergency admissions and excess cost to NHS services. It gives practices a structured way to prioritise patients who would benefit most from optimisation, helping improve outcomes at both individual and population level.

Cancer Protect enables your ICB to centrally support practices in the proactive identification and optimisation of individuals who may be at increased risk of cancer-related complications, delayed diagnosis, emergency presentation or poorer outcomes. CKD Protect enables your ICB to centrally support practices in the optimisation of individuals with, or at risk of, chronic kidney disease. These patients can experience a high burden of complications, emergency admissions and excess cost for NHS services if deterioration is not identified and managed proactively. The programme supports practices to prioritise at-risk individuals, improve monitoring and optimise care pathways, helping to slow progression, reduce avoidable complications and support better long-term renal and wider health outcomes.

CVD Protect enables your ICB to centrally support practices in identifying and optimising care for individuals at increased risk of cardiovascular disease and related complications. Patients within this cohort may have a particularly high incidence of emergency admissions, long-term morbidity and excess cost to NHS services. The programme supports proactive risk management, prioritisation of at-risk individuals and targeted intervention, helping practices reduce avoidable cardiovascular events and maximise the impact of population health management.

Diabetes Protect
Diabetes Protect enables your ICB to centrally support practices in the optimisation of individuals living with diabetes or at increased risk of diabetes-related complications. These patients can experience a high incidence of cardiovascular disease, renal disease, emergency admissions and increased NHS resource use when care is not proactively managed. The programme helps practices identify those most in need of review, optimise treatment and monitoring, and support improved outcomes through targeted, cost-effective intervention.

Live Patient Records
The Advice and Guidance (Eclipse Live) service gained NHS Digital central assurance in 2017 and is being supported by NHS central funding. The service has been commissioned by more than 2500 GP Practices and over 50 CCGs and is currently actively risk stratifying more than 27 million patients and continues to expand across the NHS. Regions utilising the service benefit from the creation of a dynamic centralised patient database containing individual patient clinical data information and proactive risk stratification.
