Patient outcomes are dependent on linking the polygenic score with 6 key parameters:
Phenotypic Associations
Lifestyle
Patient Equality Parameters
Patient Education
Patient Engagement
Patient Treatments
By combining the polygenic scores with the other 6 risk parameters, true insight into the relative and absolute risk is obtained. From this the potential for reversible risk can be evaluated.
The system has been designed to ensure interoperability with other software systems and devices. This allows as much data as possible to be gathered from as many sources as possible in order to optimise its performance and efficacy.
ECLIPSE effortlessly uses ‘big data’ to apply refined care at an individual patient level.
Cancer Surveillance Programmes
ECLIPSE gathers data from integrated care systems, effortlessly running billions of algorithms to effectively understand the impact of every clinical pathway on a patient’s health. This data collection, analysis, refinement, validation and feedback is on-going, creating a never-ending virtuous cycle of health optimisation creating impactful proactive solutions, personalised care and precision based medicine.
Healthcare needs to be redesigned to identify the underlying reasons for medical conditions and their complications. These insights enable the early identification of reversible risk with the ability to implement impactful solutions. This can be achieved using polygenic scores in combination with the healthcare records, experience of millions of patients and artificial intelligence to create and validate new impactful cost-effective pathways.
The implementation of the system at both organisational and patient levels is needed to achieve true clinical improvements. Artificial intelligence can then be utilised to identify and validate new impactful clinical pathways transforming healthcare.
Vaccination Programmes
Eclipse’s (equality of care led insights for patient safety and engagement) Technology Enabled Care solution allows us to provide vaccination services through two innovative initiatives: 1. Enhanced vaccination uptake of high-risk patients; 2. rapid detection of influenza, RSV, adenovirus, covid-19 and Strep A in symptomatic patients, through remote clinical monitoring and point-of-care rapid testing.
The system automatically identifies high-risk patients that have not yet received a COVID-19 or influenza vaccine, automatically encouraging them to receive these protective measures, through automated engagement.
The service is available for all Eclipse-activated ICSs providing centralised supply and training for all GP surgeries and care homes, together with centralised reporting and support through virtual hubs.
In keeping with Eclipse’s award-winning admissions avoidance programmes, the service will:
Significantly increase vaccination rates in high-risk individuals
Significantly reduce admissions relating to sepsis
Enhance rapid diagnosis of potentially life-threatening infections
Encourage appropriate antimicrobial stewardship
Prevent outbreaks in vulnerable individuals e.g., care home residents
Activation of the support service is fully managed by the Eclipse team and the testing kits provided are easy-to-use, provide results within 10 minutes of testing and have over 95% specificity.
The platform is fully integrated with Eclipse’s Technology Enabled Care solutions and directly supports the core20PLUS5 NHS equality programme.
