Suffolk & North East Essex AF Detect Programme

In partnership with Suffolk & North East Essex ICB, Health Innovation East, Ipswich Hospital and ECLIPSE (Equality of Care Led Insights for Patient Safety & Engagement)

Practice Pro-Forma

Project overview:

AF Detect is a secondary care, atrial fibrillation (AF) remote digital monitoring project to test the acceptability and feasibility of identified populations to self-monitor for AF via a digital Fibricheck application and wearable single lead ECG, Zio XT Patch, aiming to provide a suitable pathway for treatment without adding additional pressure on primary care.

This project is led by SNEE ICB and will be tested at Ipswich Hospital.

Eligible patients will be identified using the Eclipse system and receive digital engagement via SMS text. This will guide them to a lander page which will explain the programme and guide them to a secure NHSConnect portal where they can complete a questionnaire and consent for participation within the programme.

Patients will then be activated on the free FibriCheck application, and the data will be collected and reviewed by the SNEE ICB team.

The objectives of the programme are:

  • To increase the detection of undiagnosed patients with atrial fibrillation

  • To improve accurate triage of at-risk patients for an AF diagnosis

  • To facilitate onward appropriate clinical management of these patients

  • To evaluate the impact of the FibriCheck application in detecting AF

  • To reduce cardiovascular risk and hospital admissions associated with cardiac events that may arise from undiagnosed AF

Please complete the below pro forma to confirm participation in this programme.
