T2DAY:
Type 2 Diabetes in the Young - Patient Engagement
Practice Pro-Forma
Background:
NHS England are rolling out a world first programme to transform diabetes care to help young people (aged 18-39) living with early onset type 2 diabetes benefit from more intensive and targeted care. This involves tailored health checks from healthcare staff, and support with diabetes management, such as blood sugar level control, weight management and cardiovascular risk minimisation.
Aims and intended outcomes of the T2Day Programme:
To work with people with early onset Type 2 diabetes to provide high quality care and optimise glycaemia, cardiovascular risk, and weight, aiming to reduce long-term complications and morbidity.
To support better preparation for pregnancy in women with Type 2 diabetes, including supporting access to contraception for those not trying for pregnancy.
To help address unmet psychological and social needs and support overall wellbeing.
Supporting your practice:
Eclipse can support your practice to conduct T2Day reviews by automatically identifying eligible patients and digitally engaging with those patients to gather information and preferences via questionnaires.
Patients can be flagged for recommended interventions such as weight management programmes, diet and exercise, weight loss initiatives, medication optimisations and pre-pregnancy counselling or contraceptive support.
