Background:

NHS England are rolling out a world first programme to transform diabetes care for under 40s to help those living with early onset type 2 diabetes benefit from more intensive and targeted care. This involves tailored health checks from healthcare staff, and support with diabetes management, such as blood sugar level control, weight management and cardiovascular risk minimisation.

Aims and intended outcomes of the T2Day Programme:

  1. To work with people with early onset Type 2 diabetes to provide high quality care and optimise glycaemia, cardiovascular risk, and weight, aiming to reduce long-term complications and morbidity.

  2. To support better preparation for pregnancy in women with Type 2 diabetes, including supporting access to contraception for those not trying for pregnancy.

  3. To help address unmet psychological and social needs and support overall wellbeing.

Supporting your practice:

Eclipse can support in conducting patient reviews by: automatically identifying eligible patients for review; engaging with those patients to ensure data completeness is achieved prior to review, through online questionnaires; formal remote review of patients by a CQC-accredited team of healthcare professional (Putting Patients First).

Patients can be flagged for recommended interventions such as weight management programmes, diet and exercise, weight loss initiatives, and medication optimisations through structured medication reviews.

Women for whom pre-pregnancy counselling or contraceptive support is indicated can receive midwifery advice and support.
